PET patient Safety / Information Questionnaires

Although I have read about other isotopes the PET / CT scans at this site all utilise 18FDG. This is a glucose radioisotope. No other contrast media or drugs are used. The only contraindication is high blood glucose and everyone is tested prior to injection.
So its surprising that the patient safety / information questionnaire is the longest I've seen so far, running to 3 pages and taking 10 minutes to go through. So why so long ?.
The main reason appears to be collecting relevant medical information and history that is unique to PET scans or will help the Radiologist interpret the images.
